How will the Housing Support Team Assist You?

We have a dedicated team of housing support professionals. You’ll be matched with an experienced housing coordinator at the time of intake to support with your long term housing needs.

Who Qualifies for Housing Stabilization Services?

Anyone who:

18+ years old
Has Medical Assistance (MA)
Has a documented disability or disabling condition
Is experiencing housing instability

Can You Help Me Find Suitable Housing?

Yes, we help individuals find appropriate housing setting based on their needs.

Settings Include: Apartments, Group Homes, Provider-controlled ICS Apartments, Long Term Care Facilities, etc.

What Types of Insurances Do We Accept?

We accept most major insurances including UCARE, Health Partners, Hennepin Health and BlueCross Blue Shield
